Existing Patients

If you are currently seeing Dr. Mashayekhi at PHMG and would like to have your records transferred to her new office, please complete the Record Release form for PHMG to Dr PM (link here) and email this signed form to medicalrecords@phmg.org and patient@sdsleepdr.com to have your records transferred.

Please leave the bottom of the first page blank, if you put a date on the bottom of the first page the release will expire on the date written. Of note, if you have seen Dr. PM for Sleep Medicine and/or Weight Management, all documents related to Sleep Medicine and/or Weight Management will be transferred.

All new patient forms will be online soon. If unable to complete forms online, please arrive 20 minutes before your appointment time to complete all forms in person.
